An approval step is the one place a human authorises an agent to act, so the only safe default is to keep waiting. Approval nodes now proceed on an explicit approval and nothing else.
- An explicit approve continues the run; an explicit reject stops it
- Anything else leaves the run paused and waiting, rather than guessing
- A paused run stays resumable — an unclear response never destroys work in flight
